      • Preferred area/community Preferred language
      • Gender of your doctor (female, male, non-binary) Provider type (doctor or nurse practitioner)
   2. Find your Personal Health Care number (on your Manitoba Health card).

Eligibility

• Be 18 or older
• Have a Manitoba Health card
• Completed your taxes from the previous year.

How to apply

1. Decide if want to enroll for your lifetime or apply each year. 
   1. • If you choose the one-time (lifetime) application, this gives the Manitoba government permission to get your income earned from your last tax return each year and automatically adjusts your eligibility.
      • If you choose the annual application, then you have to submit your Notice of Assessment from the Canada Revenue Agency every year.
